I need a clean, fill-able PDF form that I can open on my iPad while I’m out in the field collecting survey data. The layout can be simple, but every field has to work flawlessly on common iPad PDF apps such as Adobe Acrobat Reader or Apple Files.
Here’s what matters:
• Text fields: single-line, plain text—no multi-line boxes or pre-filled examples.
• Checkboxes: each group must accept only one tick, so they behave like radio buttons and prevent multiple selections.
• Overall file size should stay light so it loads quickly on-site.
Once the structure is finished, send me the finished interactive PDF plus the original source file (InDesign, Acrobat, or similar) so I can tweak labels in the future. I’ll review it on my iPad; if every field activates correctly and the single-choice logic works, we’re good to go.
