I have a website template that I need customized and uploaded to my Bluehost server.
My business installs car alarms and GPS live tracking systems. I want to use this template:

I need the website adjusted to match my business style. For example:
Replace “Sendy” with my company logo
Replace the laptop image with something related to a car alarm or an armed vehicle
Adjust colors, text, and branding
Keep the homepage simple, only small changes needed
I also need a new page for forms, with categories such as:
Complaint Form
Authorization Form
Transfer Form
Service Cancellation Form
When someone clicks a form category, it should open a form similar to this:

The form page must include my logo and branding. When the client submits/saves the form, it should redirect to a confirmation page like this:

Requirements:
Store all submitted form information in a MySQL database
Send the client an HTML email confirmation
Connect everything to my existing admin page
In the admin page, I should see all form categories
Inside each category, I should see the client name and car plate number
When I click a submission, I should see all details the client filled in
Add Approve and Decline buttons
If I click Decline, a text field should appear where I can write the reason
When I press send, it should send the client an HTML email with the decline message
